The Professional, Special Services Liaison will be the liaison with external agencies to ensure coordination with the services they are providing to MVP Medicaid members. As liaison to county Foster Care Coordinators or Voluntary Foster Care Agencies, this role will act as primary
contact person to assist with enrollment, disenrollment, and access to care for the foster care children.This professional will work with MVP Care Managers, Foster Care Coordinator, hospitals, and/or Health Home Care Managers and provider network to ensure timely access to and completion of health assessments for children in foster care, and ensure smooth care transitions.

This role will also act as liaison and facilitate access to Community First Choice Option(CFCO) services to support MVP members in long term community care.Responsible for interacting with service professionals and members to supply computer generated authorizations for designated medically necessary services. This position involves telephonic outreach to members and care providers to ensure coordinated services are in place and the patient’s established care needs are met.This position will be responsible for documenting
all work including case tracking, record keeping, the coordination of member mailings, and clear and concise documentation in Care Radius.

Performs other related duties as assigned.
