Job Summary:  

Provides Hyperbaric Oxygen Therapy to all referred
patients.  Performs specific assigned wound care activities within
the State defined “Scope of Practice”; works collaboratively with all other
disciplines at assigned unit.  Uses the Nursing Process to plan,
implement, direct, and evaluate patient care to achieve optimal patient
outcomes; works collaboratively with all other disciplines on assigned unit.

Detailed Responsibilities:  

  • Conduct initial and ongoing age-specific assessments in designated
    time frames
  • Contribute to Interdisciplinary Plan of care using assessment data
  • Evaluate effectiveness of plan of care and modifies, as appropriate
  • Involve patient / family / significant other in developing plan of care
  • Collaborate with other clinical disciplines in developing plan of care
  • Provide care based on scientific principles / nursing theory and
    evidence based practice
  • Recognize change in the patient’s condition and intervene
  • Prioritize nursing actions, reflecting patient needs, acuity, and
    desired outcomes
  • Implement physician orders accurately within appropriate time frames
  • Demonstrate competency in core and unit-specific clinical skills
  • Identify continuing care needs of the patient for the discharge plan
  • In the absence of a front desk coordinator, scheduling patients for HBO therapy or wound care visit, utilizing hospital software/ systems as well as the RXH’s EMR system may be required
  • In the absence of a front desk coordinator, registration to be accomplished and in accordance with hospital policy and software / systems requirements as well as the RXH’s EMR system
  • All patient treatment notes to be entered into the RXH’s EMR system
  • Provide patient / family education to include:
    • Assessment of learning needs, abilities and readiness to learn
    • Documentation of teaching activities
    • Provision of instruction as appropriate, e.g. pain management, medication, diet,
      equipment, activities after discharge
    • Provision of appropriate educational materials, as available prior to discharge
  • Document all care provided accurately and completely according to Hospital and Department Policy
  • HIPAA Compliance – complies with all of the Privacy and Confidentiality Policy and Procedures outlined in the “Privacy, Security and Confidentiality Agreement”
  • Adheres to all Patient Safety Initiatives and Infection Control Procedures as outlined in the hospitals safety manual.  Appropriate use of Hospital resources - supplies and equipment
  • Process and utilize all computerized patient care functions, e.g. ADT, diagnostic test results, etc.
  • Scheduling for HBO therapy or wound care visit, utilizing hospital software / systems as well as RXH’s EMR system
  • Booking appointments appropriately based on time, staffing, equipment and supplies
  • Cross reference will be made to physicians’ credentials for specialty and delineation of privileges and also for a list of procedures approved for the wound care center
  • Registration to be accomplished and in accordance with hospital policy and software / systems requirements as well as RXH’s EMR system
  • Insurance and financial information provided by related hospital departments then needs to be entered into WoundDocs
  • Verify specific insurance benefits and obtain preauthorization for wound care procedures
  • Patient visit charge information to be provided by wound center personnel and provided to the respective billing and finance department or entered directly into hospital charge entry systems in accordance with hospital policies and acceptable standards
  • Perform daily patient schedule: charge entry reconciliations and account for all outstanding superbills


Education and Technical Skills:

  • Must have and maintain a valid CPR certification from the American Heart Association
  • Valid LPN license
  • CHT trained/certified preferred 
  • Must have prior experience working with wound care patients
  • Must have basic skills in Microsoft Word, Outlook and Excel
  • Prior experience with an Electronic Medical Records (EMR) system preferred
  • Must have and maintain a valid Licensed Practical Nurse (LPN) license in the state in which the Nurse will work
  • Prior experience with billing and coding preferred
