The mission of the Project HOME community is to empower adults, children, and families to break the cycle of homelessness and poverty, to alleviate the underlying causes of poverty, and to enable all of us to attain our fullest potential as individuals and as members of the broader society. We strive to create a safe and respectful environment where we support each other in our struggles for self-esteem, recovery, and the confidence to move toward self-actualization.
Project HOME achieves its mission through a continuum of services comprised of street outreach, a range of supportive housing, and comprehensive services. We address the root causes of homelessness through neighborhood-based affordable housing, economic development, and environmental enhancement programs, as well as through providing access to employment opportunities; adult and youth education; and health care.
Project HOME is committed to social and political advocacy. An integral part of our work is education about the realities of homelessness and poverty and vigorous advocacy on behalf of and with homeless and low-income persons for more just and humane public policies.

Essential Duties and Responsibilities
- Welcome and support all individuals who come into the Hub, and uphold a standard of a clean, safe, and respectful environment.
- Maintain situational awareness at all times to prevent or quickly respond to incidents or crises that may occur, ensuring a safe environment for all
- Build respect and rapport to establish ongoing relationships with individuals who frequent the Hub
- Maintain regular contact with individuals on caseload, and discuss short and long-term goals with individual and develop a plan to meet them
- Assess and refer individuals for Living Room membership and programming
- Conduct assessment, referral and provide follow-up services to individuals in appropriate treatment, residential, and other relevant sites
- Know individual’s residential and treatment history. Discuss what worked, what didn’t and why.
- Identify and prioritize needed services and target most appropriate facilities for placement
- When placement is achieved, continue to work with individual and service providers to maximize support and minimize opportunities to leave continuum of care prematurely
- Maintain close contact with individual throughout placement and provide support until suitable plan for more permanent service/housing is implemented
- Discuss individual’s concerns, fears, and frustrations, and work to resolve or alleviate while challenging the individual to address potential obstacles they may face in their long-term goals
- Complete housing applications for individuals as appropriate
- Coordinate referrals for targeted and intensive case management, substance use, and mental health services including: assessing the immediacy of the participant’s need, level of service required, and appropriateness of service
- Advocate for individual’s needs as necessary within larger system of services
- Assist individuals with obtaining ID, benefits, and medical and psychiatric evaluations for placement
- Keep careful paper and electronic records of participants’ progress, and document visits/plans in various systems, including ETO and HMIS (training on systems will be available)
- Maintain case records, file system, and complete all necessary report forms, progress reports, and service plans
- Utilize headset to maintain communication with staff throughout the Hub
- Review log book at the beginning of each shift, update as necessary during shift to keep communication flow between staff, and include a brief summary of the shifts happenings before clocking out, and communicate directly with relief staff during shift overlap period to ensure a seamless transition between shifts
- Participate in weekly staff meetings, being prepared to provide updates about caseload progress
- Other duties as assigned
- 4-6 years of case management and drug and alcohol services
- Demonstrated knowledge of and sensitivity to working with people living with mental illness, substance use disorders, and/or in recovery
- Strong computer skills and demonstrated experience with electronic charting systems and social service documentation standards.
- Experience in the social service system and the homeless provider network
- Strong interpersonal skills
- Willingness to learn, and to listen and respond to others with compassion
- Ability to build trusting relationships with people from diverse backgrounds and interpret cross-cultural influences in a helpful manner
- Excellent listening, communication, collaboration, and teamwork skills
- Ability to deal effectively with resistance
- Sense of humor
- Experience with the mental health system in Philadelphia preferred
- Valid Driver’s License
