Job Summary: The Health Services Coordinator will be a key member of Prevention Point Primary Care (PPPC) team, collaborating with the Prevention Point social services team to connect vulnerable individuals to housing, rehabilitation and health services. Healthcare at Prevention Point will not only addresses acute conditions of vulnerable individuals, but it will also provide preventative health care and chronic disease management. PPPC will also be used as an engagement tool and a way to access assessments needed for housing.

Project Home partners with Prevention Point to create the Prevention Point Primary Care team which will function as a part of an urban federally qualified health center (FQHC), Steven Klein Wellness Center. The Coordinator will schedule for, work closely with, troubleshoot with, and provide administrative support to health care providers who are delivering health care services Prevention Point. The Coordinator will also be responsible for tracking and reporting activity at the PPPC.

Essential Duties and Responsibilities:

  • Schedule and check in patients for appointments at the PPPC
  • Verify eligibility for each patient who is scheduled at PPPC
  • Assist patients with changing Primary Care Providers and/or health insurance coverage as necessary in order to facilitate billing for visits.
  • Assist with health insurance matters including selecting Medicaid MCO, assisting with changing primary care providers, selecting Medicare advantage plans, and completing new Medicaid applications.
  • Help coordinate and schedule services for other social services offered at Prevention Point
  • Assist patients with scheduling with other Primary Care Providers per patient preference
  • Coordinating referrals and in-house follow-up appointments
  • Maintain and monitor a daily sign-in sheet for health services
  • Engage with PPP visitors around healthcare needs; greet visitors who arrive via appointment or walk-in to see a health care provider
  • Complete brief intake on people requesting health care services, including obtaining signed consents to share health information between health care providers and agencies
  • Conduct a focus group of patients and conduct a written survey for patients re: quality and accessibility of health clinic services on an annual basis
  • Facilitate any follow-up appointments needed at off-site locations for primary and specialty care
  • Maintain accurate records in the Nextgen software, produce clinic reports on a regular basis, and contribute to larger PPPC reporting
  • Administrative duties as required including maintaining individual health services charts on patients served, copying, and faxing.
  • Ordering and tracking medical supply and medication inventory as needed for the PPPC location.
  • Participate in regular planning/coordination meetings with other PPPC and health care providers
  • Work with licensed health care volunteers and other volunteers.
  • Maintain situational awareness at all times and use de-escalation strategies to maintain a calm and welcoming environment, conducting individual interventions as needed
  • Based on PPP schedule and need at other locations, provide back-up front desk, benefits counselor, or referral coordinator support to other PHHS locations.
  • Other duties as assigned

Minimum Qualifications

  • BA/BS degree with 1 year experience working with mental illness/dual diagnosis, and supportive housing or Associates degree in related field with 3 years related work experience or GED/High School Diploma with 5 years related work experience. Prior healthcare experience and/or prior experience interacting with individuals experiencing homelessness.

Preferred Qualifications

  • Interest in and/or lived experience with mental health issues, substance use and recovery, homelessness, and healthcare for vulnerable populations
  • Prior experience in one of the following roles at a healthcare facility: referral coordinator, front desk, social worker, benefits counselor
  • Excellent communication, collaboration and team work
  • Willingness to work with people from diverse backgrounds
  • Ability to establish a rapport and put patients at ease
  • Knowledge of (or ability to quickly learn) community-based resources for addiction, behavioral and physical health in Philadelphia
  • Excellent time management, organizational skills, and attention to detail
  • Excellent computer skills (experience w/Microsoft Word, Excel, Outlook, etc.), ability to use (with training) Nextgen Practice Management and EHR system.
  • Ability to successfully manage several tasks simultaneously;
  • Ability to enjoy a high-performing, fast-paced environment
  • Medical background preferred by not required
